The role is responsible for providing advanced nursing and quality client centred nursing care services to Aboriginal, Guardianship of the Minister and vulnerable young people.As a Community Nurse, You Will Be Responsible ForLeading the development and meticulous documentation of comprehensive multidisciplinary casework services, encompassing thorough intake, assessment, and physical and psychosocial health screening, ensuring effective treatment protocols that deliver holistic care tailored to individual needs.Delivering expert social support, advocacy, and referral services to vulnerable young people, empowering them to navigate and access essential primary health care solutions, while promoting their overall well-being and long-term stability.Utilizing data analysis and evaluation to monitor clinical service delivery, contributing insightful reports that inform decision-making and identify key opportunities for continuous improvement, enhancing the effectiveness and efficiency of care provided.Collaborating in the review, development, and seamless implementation of best practice changes that elevate service delivery standards, while nurturing a culture of continuous learning and professional growth within MYH, ensuring sustained excellence in care.What WCHN Can Offer YouRelocation financial support may be negotiated, subject to eligibility.Salary package a range of benefits (including living expenses such as mortgage, rent, utility bills and groceries) up to $9,010 each FBT year + $2,650 for Meal Entertainment expenses each FBT year.Access to Fitness Passport (workplace health and fitness program that gives you and your family access to a wide choice of gyms and pools across South Australia.
