The Role:
We are dedicated to supporting individuals in understanding, implementing, and maximizing their NDIS experience. As a Support Coordinator and Recovery Coach, you will play a key role in alleviating stress and ensuring effective support delivery. Your responsibilities will include:
* Implementing all supports outlined in NDIS participant plans.
* Cordinating services within the community to ensure effective support delivery.
Key Responsibilities:
Plan Implementation and Coordination:
* Implement Supports: Implement all supports outlined in NDIS participant plans.
* Community Coordination: Coordinate services within the community to ensure effective support delivery.
Operational Knowledge:
* NDIS Knowledge: Maintain operational knowledge of the NDIS and its interface with service delivery in the Disability Sector.
Communication and Relationship Building:
* Effective Communication: Communicate effectively with stakeholders, clients, and community members.
* Relationship Building: Build strong relationships with clients and their families, as well as community and service providers.
Community Linkages and Stakeholder Engagement:
* Linkage Services: Link clients and their families to community and service providers.
* Stakeholder Meetings: Participate in stakeholder meetings to contribute to effective service coordination.
Record Keeping and Reporting:
* Client Records: Keep accurate and up-to-date client records on the MCCS client management system.
* Reporting: Submit reports in relation to clients and programs as required.
Professional Development and Risk Management:
* Professional Knowledge: Develop and extend professional knowledge on NDIS requirements and options.
* Risk Assessment: Follow ongoing risk assessment procedures.
Support and Capacity Building:
* Support Delivery: Support the implementation of all NDIS participant plan supports within the community.
* Capacity Building: Build strong, respectful relationships promoting capacity and resilience during complex challenges.
* Clinical Coaching: Coach individuals to increase recovery skills and personal capacity.
Collaboration and Plan Management:
* Partnership Work: Work collaboratively with participants, families, carers, and other services to design and adjust recovery plans.
* Participant Engagement: Support participant engagement with the NDIS, applying, monitoring, and reviewing outcomes.
* Budget Management: Manage participant budgets.
Essential Attributes:
* Working Knowledge: Working knowledge of the NDIS.
* Mental Health Experience: Significant experience in the mental health field, with lived experience highly recommended.
* Driver's License: Driver's license with reliable transport.
* Security Clearance: Security check requirements including police clearance, 100 points ID, NDIS Worker Screening Check, and rights to work in Australia.
* Vehicle Requirements: Maintained vehicle with minimum third-party insurance.
* Covid-19 Immunization: Covid-19 immunization certificate.
* First Aid Certificate: Current First Aid/CPR Certificate, NDIS Worker Orientation Certificate, NDIS Supporting Effective Communication Certificate, and Covid-19 Infection Control Training.
Qualifications and Experience:
* Suitable Certification: Suitable certification or relevant experience.
* Experience: At least 2 years' experience in a similar environment.
* Skills: Understanding of mental health and physical health issues, Strong communication, negotiation, and conflict resolution skills, Ability to work unsupervised and within a team environment, Experience in building and maintaining effective relationships with support organizations, Ability to work with various stakeholders including clients, carers, GPs, and support organizations, Willingness and ability to travel as part of the role, Current driver's license, reliable vehicle, police clearance, and working with children check.
Salary: $70K - $90K PA
