What our Service Coordinators day-to-day looks like: Provide a centralised planning and coordination Regular communication with participants and families Conduct reassessment of care plan for participants Arranging and coordinating support services Monitor plan outcomes and expenditure Ensure mainstream services meet the obligations to the customer (Housing, education, justice, health) Record keeping of information, time and billing Maintain organisational documentation Respond to NDIS/Home Care Package/Aged care enquiries Candidate requirements: Cert IV in ageing disability OR diploma in community services Valid drivers license NDISWC First Aid certificate Job Type: Part-time Flexible working arrangement available Expected hours: 30 per week Schedule: Monday to Friday Application Question: Do you have requirements to work in Australia?
