Social Worker Grade 2 - Rural Allied Health TeamGV Health is the main healthcare provider in the Goulburn Valley region, servicing a broad catchment area of approximately 120,000 people.We are the largest permanent employer in the region with over 2,600 employees spread across our five main sites.To meet the evolving healthcare needs of our community we have completed Phase 1 of our redevelopment project which includes a five-storey Inpatient Unit building, expanded theatre capacity, new Critical Care Unit, a state-of-the-art Dialysis Unit and an extension of the current Emergency Department.About the role Come and join the GV Health Rural Allied Health Team (RAHT), a team with a well-renowned supportive workplace culture with an opportunity to work alongside experienced Allied Health Professionals.In this role you will work autonomously, undertake a varied and diverse workload and provide interventions in predominantly home-based settings, making a true difference to our client's independence and well-being.There is no nursing home work.Our multidisciplinary team includes Dietetics, Occupational Therapy, Physiotherapy, Podiatry, Social Work and Speech Pathology services, supported by Allied Health Assistants.The Social Worker will provide person-centred care with an opportunity to:Travel across the Goulburn Valley region working in home-based settings.Work with clients, carers and families to provide support, counselling, information, advocacy advice and referral with a focus on informed decision making and empowerment solutions to promote independence at home and in the community.Develop goals and care plans to assist clients to remain at home and active in their local community.Advise on strategies and areas of assistance to support clients to remain living independently at home and within their local communities.Work with a diverse range of clients and health conditions.Participate in student training programs.We are looking for Bachelor of Social Work or Master of Social Work (qualifying) and a member of the AASW.Broad knowledge in the planning, implementation and evaluation of current Social Work practice within health care settings including home-based and community settings.Experience in the delivery of Social Work services to clients with complex presentations, inclusive of mental health problems, complex health conditions, cultural and diverse backgrounds and social disadvantage.Experience in providing services within a self-management framework and active service model/reablement approach to care delivery.Experience working with frail older people, people with disabilities and their carers, and people living in rural communities.A comprehensive orientation program is available.In your application, you are required to provide responses to the key selection criteria and provide a copy of your AASW membership.All appointments are made subject to satisfactory police check and Victorian 'Employee' Working with Children's Check.Completion of a Commonwealth of Australia Statutory Declaration is required prior to commencement.GV Health is an equal opportunity employer and is committed to attracting and retaining a diverse workforce that reflects the community we serve.Applications from Aboriginal and Torres Strait Islanders are encouraged. #J-18808-Ljbffr
