Your Opportunity:
The Nurse Manager – Navigate Your Health is a clinical and service leader, accountable at an advanced practice level for the effective coordination of healthcare navigation and caseload management for children and young people in child safety and youth justice populations, meeting patient and organisational requirements across the primary, community and secondary/tertiary healthcare continuum. This will be done in partnership with Child Safety, Youth Justice, General Practice and Aboriginal and Torres Strait Islander Medical Service partners.
The role is responsible for the facilitation and provision of quality and cost-effective clinical services which are consistent with contemporary standards and best practice. The Nurse Manager will utilise data and information to ensure that the service is organised for safety and efficiency, and key performance indicators are in place to capture the standard of service delivery.
