Directions Health Services is an innovative non-profit organisation that has provided specialist alcohol and other drug (AOD) services in the ACT and NSW for more than 40 years. Alongside is one of Directions’ Canberra based programs. Alongside provides non-clinical psychosocial support to clients experiencing co-occurring mental illness, substance use and other comorbidities, who do not have access to NDIS or other capacity building supports.

Our Directions’ team includes Counsellors, Social Workers, a Psychologist, Chemists, GPs, Registered Nurses, and a Psychiatrist. About the position
The Advanced Practice Nurse is responsible for providing advanced comprehensive nursing assessment and support to at risk people with complex health problems, and their families, in a Primary Health Care setting, including outreach service delivery. For Teen 25 they are responsible for providing advanced, comprehensive, nurse led and opportunistic primary health, sexual health, AOD and mental health interventions for young people at risk, using a harm minimisation framework.
This is a permanent part-time position.
Responsibilities
* Provide comprehensive Primary Health Care to centre-based and outreach clients, whose health is impacted by current or past alcohol, tobacco and other drug use, mental illness, or other health concerns, including health promotion and prevention activities, screening, early intervention, acute care and chronic illness support.
* Provide comprehensive Primary Health Care for young people in the Teen 25 space including screening, health promotion and prevention, education, acute care and appropriate and timely referral.
* Work collaboratively with Althea team members and other Directions staff members to support a comprehensive and holistic approach to meeting clients’ needs and participate in case reviews as required.
* Support the Althea clinic and outreach operations through general practice coordination and stock management activities, for example cold chain vaccine management and other activities as per the Althea practice manual.
* Support clients to access medications, including liaising with, and attending pharmacies, accessing brokerage where necessary, and at Directions’ request transport medications (including S8 medications such as Buvidal).
* Ensure documentation is up to date and of an appropriate standard, including client case notes, reports and other written communication, and ensure effective handover of information when required.
* Maintain professional and clinical standards in relation to service delivery and confidentiality.
* Take responsibility to work in accordance with health and safety guidelines and Directions’ policies and procedure and actively engage in continuous quality improvement and risk management processes.
* Be accountable for coordinating, managing, and providing clinical care and working under standing orders as required.
* Identify and contribute to ongoing quality improvements, including development of protocols.
* Undertake other duties as directed, within the approved scope of clinical practice.

What we would like you to have
* Registered General Nurse with current Australian Health Practitioner Regulation Agency registration.
* Minimum of 6 years nursing experience, preferably in a Primary Care setting, including experience working with at risk communities or in a community or general practice setting.
* Demonstrated engagement with relevant professional bodies.
* Demonstrated record of continuing professional development.
* Proven clinical competency in undertaking general health assessments including immunisation, venepuncture, pathology specimen collection, wound and vein care.
* Demonstrated advanced nursing clinical knowledge, skills and experience and extended practising clinical assessment and care management with the ability to prioritise and undertake timely, comprehensive, and evidence-based assessments.
* Post-graduate experience in an appropriate field such as mental health, sexual and reproductive health (including a relevant qualification for cervical screening) and/or primary health care.
All appointments will be conditional upon having a Working with Children Check, current First Aid and CPR certification and a satisfactory National Police Check.
