As a registered NDIS provider, New Horizons specialise in supporting people with disability, mental health concerns, those who are aged, people at risk of homelessness, humanitarian entrants, youth, and Indigenous Australians.
You will be the first point of contact for NDIS participants, families and Support Coordinators who are seeking information about New Horizons or who have chosen us to provide services for them under an NDIS plan.
Based in Port Macquarie, part-time position, 24 hours per week Support Worker role working Monday to Sunday, providing the highest quality in high needs / complex care Mental Health & Disability services to the community.
The Day to Day:
1. Meet the daily care needs of residents within the community
2. Provide support to residents presenting with complex care needs
3. Tasks may include but are not limited too;
4. Accompanying to appointments, social & recreational activities, adhering to specialist plans and medication administration, personal care, manual handling, meal preparation as per NDIS plan goals
You are:
5. Invested in providing the highest quality of Person Centred support services to the community.
6. Competent in working with people with a disability and or Mental Health diagnosis
What you need to have:
7. Use of own vehicle
8. Appropriate certificate level qualification in Community Services, Mental Health or Disability, OR equivalent relevant experience in the human services industry
9. Valid NSW Drivers C class licence, with ability to drive vehicles in licence class, including large cars and vans.
10. Must be fit to undertake the physical requirements of customer support, including the capacity to lift 10 kilograms unassisted
11. Intermediate skills in Microsoft Office, other common systems
12. Passion, Respect & Integrity
Ideally you will also have:
13. Advanced Solid knowledge of services and programs under NDIS
14. Understanding of Person-Centred Practices
