Seeking passionate and organised individuals to play a pivotal role in coordinating and delivering outstanding clinical care within our dynamic Specialist Community Mental Health Service. Responsibilities: Deliver comprehensive clinical care coordination: Facilitate communication and collaboration between patients, clinicians, and community stakeholders Develop diverse professional portfolios: Gain experience in depot and clozapine clinics, supporting consumer fitness groups, and facilitating therapeutic interventions. Conduct thorough assessments: Utilise your expertise to perform mental health and psychosocial assessments, risk assessments, and develop personalised treatment plans. Support team leadership: Collaborate with the Team Manager to contribute to a cohesive and efficient team environment. Contribute to quality improvement: Engage in clinical auditing, research, and evaluation initiatives to continuously improve patient care outcomes and service delivery.
