Key Responsibilities
The Case Manager will oversee a portfolio of Home Care Packages clients, working with the rostering team to schedule carers and collaborating with the Frontline Team Leader to ensure best possible care.
The role involves managing issues, mitigating risks, and providing exceptional care to clients, identifying potential risks and implementing processes to enhance safety.
* Sound Aged Care and Home Care Knowledge with previous Case Management experience.
* Highly developed communication and relationship management skills.
* Exceptional problem-solving skills and ability to relate with those navigating the Aged Care system.
Requirements

* A current driver's license and reliable vehicle.
* COVID vaccination & booster.
