A unique opportunity to contribute to a Dementia Centre initiative, supporting individuals living with dementia as they transition from hospital settings back into their homes or residential care.

Key Responsibilities:
* Facilitate on-site support and implement relationship-based support strategies, modeling and coaching clients, carers and care staff.
* Collaborate with peers to develop aged care staff and carer skills, expertise, and applied knowledge in understanding individuals living with dementia and providing best practice support and care.
* Support implementation of recommendation plans, utilizing modeling and coaching techniques, and seek guidance from Team Leader, Dementia Consultant and Senior Consultant as needed.
* Foster and support trial and implementation of brokerage items with clients and their carers, seeking guidance from Team Leader, Dementia Consultant and Senior Consultant as required.
Requirements:
* Demonstrated organizational abilities, including effective time management and prioritization in a busy work environment with competing demands.
* Ability to work independently and as part of a team.
* Commitment to delivering exceptional customer service, incorporating conflict resolution principles.
* Current driver's license and safe driving record.
* Willingness to travel intrastate and interstate for up to five days at a time to support DC service delivery.
* Flexibility to work varying hours as needed to meet client and carer needs.
* Relevant qualification: Allied Health Assistant, Diversional Therapy, Enrolled Nursing (EN), Aged Care, Cert III or IV in Individual Support / Leisure and Health, or similar highly desirable.
