The Clinical Team Leader reports to the Clinical Manager and works collaboratively with various teams across the organization. This role will see you play a key role in coordinating clinical care for complex, high-risk customers and ensuring that recommendations align with clinical best practices and HomeMade's strategic objectives.
This is an exciting leadership opportunity to drive excellence in clinical care while leveraging digital health innovations, including AI, to enhance customer outcomes and operational efficiency. The role requires a solid understanding of fee-for-service clinical support and the evolving aged care landscape, particularly the Aged Care Reforms, including Support at Home and the strengthened Aged Care Quality Standards.
Key Responsibilities:Clinical LeadershipCoordinate and oversee clinical care and recommendations for complex and high-risk customers, ensuring person-centred, safe, and high-quality outcomes.Provide subject matter expertise on clinical governance, aligning with the Aged Care Act and the Aged Care Quality Standards.Provide support to the Clinical Nurses and serve as an escalation point for complex issues, liaising with internal stakeholders to ensure timely resolutions.Collaborate with the Clinical Manager to develop strategies that support clinical team effectiveness and continuous improvement initiatives.Monitor reporting on the efficiency and productivity of the clinical team, maintaining a focus on effective time management and delivery of required outputs and service levels.People & LeadershipSupervise, coach, and mentor Clinical Nurses to build a high-performing team environment.Communicate clear performance goals and conduct regular reviews to foster accountability and professional growth.Manage team resources effectively to meet clinical priorities and maintain an inclusive and supportive work culture.Collaboration and InnovationWork with cross-functional teams to promote service innovation and implement contemporary best practices.Actively participate in organizational planning, sharing insights on clinical priorities and customer needs.Continuous ImprovementIdentify and address gaps in clinical care through audits and feedback, implementing corrective actions and quality improvement measures.Contribute to the refinement of policies, procedures, and standard operating guidelines to ensure compliance and best practices.Collaborate with the Clinical Manager to identify, implement and evaluate the success of measures to improve efficiency and productivity for the clinical team.Clinical GovernanceUphold the service delivery of the Clinical Team in accordance with the Aged Care Act and Aged Care Quality Standards.Work with the Clinical Manager on future process change, risk assessment, and documentation.Support the Clinical Manager in monitoring the standards of clinical support within the growing HomeMade team.Work with the wider HomeMade team to recognize opportunities for service innovation and take a proactive approach to implementation.Participate in, contribute to, and implement quality improvement and risk management into all aspects of services including policies and standard operating procedures.Skills & ExperienceCurrent AHPRA registration as a Registered Nurse.Experience in case management and/or management of clinically complex cases.Proven experience in leading or coordinating clinical teams, including coaching and mentoring for performance.Strong understanding of the aged care sector, particularly home and community care services.Familiarity with the Aged Care Quality Standards and related reforms, including Support at Home.Knowledge of profitability within fee-for-service clinical support.Interest or experience in digital health initiatives, including AI in healthcare.Excellent problem-solving and critical-thinking skills, with a strategic mindset.Who you will be working withYou would be joining our team, a group of people who know they can make a difference to their customers.
