Whether inherited, present from birth, or acquired, these conditions require a holistic approach to care, with families playing a central role in effective management.Working as part of a multidisciplinary team, you will empower families to navigate the complex medical, emotional, and social challenges associated with chronic conditions, helping them to build confidence, skills, and strategies for effective self-management and overall well-being.You will help families to identify and minimize emotional and logistical challenges of caregiving, minimizing barriers to effective self-management, and enabling children and their families to manage their condition at home, school, and through changes in their child's care needs.You will also play a vital role in supervising staff and students, contributing to professional development, and advancing best practices in the field.Key responsibilities include:Family Empowerment: Equip families with the knowledge, skills, and resources to self-manage their child's condition effectively, fostering health literacy, emotional resilience, and day-to-day care challenges.Multidisciplinary Collaboration: Work as part of a multidisciplinary team (MDT) to address the broader physical, emotional, and social needs of patients and families, ensuring a holistic approach to care.Transition Support: Assist families during critical transition points, such as starting school, adolescence, or other significant life phases, helping them adapt to changing needs and challenges.Please note a tertiary qualification in Social Work and eligibility for full membership of the Australian Association of Social Workers is a requirement for this role.Resources can be provided to support transition from an adult care environment.The ideal candidate will have advanced clinical Social Work experience, particularly in pediatric or chronic health care settings.About our Department:Based at Perth Children's Hospital, our dedicated team of Social Workers, Aboriginal Health Liaison Officers, and Welfare Officers offer a wide range of services to inpatients and outpatients from birth to 16 years of age.Designed to assist families in managing the practical, personal, and emotional impacts of illness, disability, and hospitalization, our comprehensive services include specialist counseling and crisis assistance, advocacy for patient needs, education, liaison with other services, and referral to community-based resources.Health issues can deeply impact the emotional and social well-being of children and their families.
