Employment Type: 2 days/week
Wage: $41- $44 hourly
Mandatory Qualification & Experience: Relevant Diploma, AND/OR Minimum of 5 years relevant experience.
Position reports to: Team Leader
Position Summary
The role of the Intensive Outreach Practitioner is to provide an elevated level of assessments and recommendations identifying ongoing support necessary for continuing case management support for children and their families. As well as facilitate and maintain an intensive level of advocacy and interventions, ensuring the child’s and their family’s needs and case management goals.
Accountabilities
* Partnering with the family to conduct initial assessments when required
* Provide an intensive range of case management support
* Have an elevated knowledge of relevant services and programs across the community sector
* Ensure the child and family’s voice is heard and that their views and experiences are captured on documentation
* Demonstrated ability to undertake and complete comprehensive risk assessments that identify potential risk, immediate or future harm to the child/ren - and vulnerable adults in the home
* Prepare and present intensive-level reports that can advocate
* Assist those experiencing domestic and family violence to identify their rights and advocate for their and their children’s needs as required
* An elevated level of understanding of the Child Protection System in Queensland
* Ability to organise, supervise, observe and analyse family time (contact) between children and their parents by Court Directives or orders
* Have an in-depth knowledge of Trauma Informed Practice to lead and support parents and be trained to deliver DFV and other programs/interventions where needed
* To provide support and advice to parents and young people
