An exciting opportunity exists for a highly functioning multidisciplinary team member, to deliver high level Social Worker clinical services and leadership across one or more specific work units, to stakeholders/customers of the Sunshine Coast Hospital and Health Service and Queensland Health. The Acute Care Team provides specialist triage, mental health assessments and provide care planning with consumers in the community setting.
Sunshine Coast Hospital and Health Service (SCHHS) is in southeast Queensland and extends through the coastal and hinterland areas from Caloundra in the south and Gympie in the north.

Key Responsibilities include:
1. Deliver independent high level Social Worker clinical practice with infrequent need for direct clinical practice supervision. Exercise professional judgement to a complex caseload, adapting routine clinical practice based on valid and reliable evidence.
2. Provide high level clinical advice, education and influence towards a common patient-centred vision and direction to professional operational teams and relevant stakeholders regarding service delivery.
3. Provide leadership oversight of ACT multidisciplinary team on a daily basis and clinical practice supervision to less experienced Social Worker practitioners and students as delegated.
Remuneration value up to $152,776 p.a., comprising salary between $124,435 - $133,901 p.a., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HP4).
