About Us?
The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.
The Staying Deadly hubs aim to provide comprehensive assessment treatment and assertive outreach support to clients presenting with mild, moderate and/or severe mental health needs. Working in a unique multidisciplinary care team as an extension of the IUIH comprehensive model of care, along with integration with the Staying Deadly Hub teams to provide appropriate and responsive care to clients would otherwise not access support or require inpatient care.
About the Role?
Staying Deadly Care Coordinators provide outcome-focused health, social, and practical support to Aboriginal and Torres Strait Islander clients, with a strong emphasis on psycho-social well-being. They work closely within a multidisciplinary team alongside clinicians to address the mental health needs of clients who have moderate to severe conditions. These clients may be at risk of entering the acute care system if their symptoms are left unmanaged, or may be transitioning from inpatient care, requiring ongoing support in the community. The role involves providing holistic, wrap-around primary health and mental health care to ensure clients are well-supported in their recovery journey. Care Coordinators manage a caseload of clients with diverse needs, working to empower them by fostering strength, resilience, and well-being. This is achieved through collaborative case management, comprehensive planning, and coordination, alongside the clinicians to ensure an integrated and supportive care experience.
Day to Day Duties may Include:
* Work collaboratively as a member of a multidisciplinary team to provide integration and coordination between different programs.
* Developing and maintaining support plans in conjunction with clients and relevant service providers.
* Work with clients in a trauma informed, recovery and/or strengths-based approach to formulate, implement and review well-being plans including a comprehensive assessment of need and safety planning.
* Assisting in the coordination and delivery of services for clients identified with drug and alcohol, mental health, chronic disease and social health issues.
* Participate in care coordination activities to fulfil needs of assigned clients transition care plans, including coordination meetings with relevant external stakeholders.
* Working flexibly in response to client and family/carer needs
* Conduct home, community and/ or custody visits to support ongoing client recovery.

* Current Blue Card
* Current Criminal History Check
* Current CPR and First Aid Certificate (Willing to obtain)
* Current C Class Drivers Licence (Qld)
* Certificate IV or Higher in relevant field
Desirable, but not mandatory:
* Demonstrated experience working with Aboriginal and Torres Strait Islander people and /or communities
