The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people's homes, other community venues as well as virtual care (telehealth). Our Lawson Community Health team is seeking to recruit a Registered Nurse. The Community Health Care Registered Nurse (RN) provides a range of services to individuals, carers and families, with the aim of maintaining and/or improving their health. Additionally, you will be responsible for the provision of high quality nursing assessment, planning, implementation, and evaluation of care using evidence based best practice guidelines underpinned by the principles of Primary Health Care. These strategies are conducted as part of a collaborative partnership with clients, families/carers and other key stakeholders (including the client's GP). The Community Health Care RN will be working on a 7 day rotating roster and will help ensure care is delivered in a person centred way using a flexible, responsive, wellness and reablement approach in line with the Australian Aged Care Quality Standards. You will be working in a multidisciplinary team in planning, implementation, assessment and evaluation of patient care. Uniform is required.
