The Transition to Acute Care Rotation Program has been developed to support the Registered Nurse to develop the skills and attributes required to work and thrive in an Acute Care Hospital.
The program offers Registered Nurses the opportunity to rotate through multiple departments and specialty units over a 10 month period whilst being supported by a designated Clinical Nurse Educator.
The program is designed to assist the Registered Nurse during the transition phase from generalist nurse or new graduate to specialist Acute Care Nurse.
DUTIES: Perform comprehensive and accurate nursing assessment of patients.Identify and implement discharge planning requirements.Plan appropriate care in consultation with both the patient and the multidisciplinary team.Evaluate patient progress and revise nursing interventions where required.Complete accurate documentation of nursing care in patient's medical record and communicate to other team members.Protect the rights of patients while respecting their values, customs and spiritual beliefs.Function in accordance with legislation and policy relevant to practice.Act in accordance with the profession's code of ethics and the professional standards of a Registered Nurse.Maintain Confidentiality.ABOUT YOU: Registered Nurse with current Authority to Practice by the Australian Health Practitioner Regulation Agency (AHPRA).Demonstrated recent clinical experience in general nursing.Demonstrated commitment to own personal and professional development and the desire to continually improve patient care.Demonstrated ability to work unsupervised and as an effective member of a multidisciplinary team.Demonstrated effective communication, interpersonal and time management skills, with the ability to work proficiently during periods of high demand and activity.Demonstrated computer literacy and ability to use healthcare software.HOW DO YOU APPLY?
