Job Summary:
We are seeking a skilled Clinical Needs Assessor to join our Aged Care Assessment Service in Bowral Hospital. As a key member of our team, you will provide complex aged care assessments using the Integrated Assessment Tool (IAT) addressing various aspects of older people referred for assessment.
Key Responsibilities:
* Conduct comprehensive aged care assessments using the IAT
* Formulate support plans that meet the needs of older persons and their carers
* Work collaboratively with multidisciplinary teams to achieve optimal outcomes
* Develop and maintain effective relationships with clients, families, and healthcare professionals
Requirements:
* Tertiary qualifications in a health-related discipline directly related to health, aged care, or a related specialist area
* Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA) or part of or eligible to be part of a relevant professional association
* Willingness to undertake Transition Training during the Program Transition Period
* Completion of training requirements as specified under the My Aged Care Workforce Learning Strategy 2023
* Minimum one year demonstrated experience in delivering services in aged care settings and/or to aged persons
