About the Young Stroke Service: The Young Stroke Service is funded by the Medical Research Future Fund and led by the Florey Institute. Through co-designed research, we are implementing a service dedicated to young adult stroke survivors. A pilot clinical hub has been established within Austin Health, geographically based at the Melbourne Brain Centre in Heidelberg, as well as via telehealth. Patients across Victoria including in remote and rural areas are able to access the clinic. The appointed Austin Health Young Stroke Service Clinical Therapist will be based at the Florey Institute, Heidelberg, Victoria. About the role: A regular day will see you: Provide outpatient clinic services for young adult stroke survivors Lead Young Stroke Service multidisciplinary meetings and provide coordination of young stroke services for patients. Support the further development of systems, processes and connectivity that underpin the Young Stroke Service. You’ll join a friendly and supportive team who genuinely enjoys their work and the contribution they make. About you: This role requires you to have: Current registration for practice with AHPRA (Australian Health Practitioner Regulation Agency) as a Physiotherapist. At least 5 years clinical experience in allied health working with stroke or chronic conditions. At least 2 years project management experience in a health-related field and/or experience with establishing new health care systems or innovating current health care systems in a patient-facing role. Demonstrable high-level patient management and communication skills. Experience in working with clinical networks and Health Departments. Experience with web-based systems and processes for electronic data capture in healthcare and knowledge of medical terminology. Experience in writing/developing data collection tools, clinical checklists, reports, protocols, standard operating procedures and policies.
