Clinical Nurse Consultant - Post Acute Care (Grade 4) Hub of medical precinct in Parkville, Melbourne Excellent employee benefits including salary packaging Full Time, Fixed Term position for 12 months Base salary for this position is from $113,360 per annum, plus superannuation About the Role This is a 12 month Fixed Term Full time position at 80 hours per fortnight within the Complex Care Hub- Post Acute Care program. The PAC CNC utilises and shares specialist knowledge and skills to faciliate safe and timely discharges of patients from the RCH to the community setting. This role involves close collaboration with the multidisciplinary team to educate patients and families, on organisational processes which support discharge. The PAC CNC provides expert clinical knowledge and skills, in the assessment and management of patients within area of specialisation. About Post Acute Care The Post Acute Care service is part of the Complex care Hub, which is available to patients who have been admitted to the Royal Children's Hospital. The Post Acute Care service specifically focuses on providing short term supports (up to 4 weeks) at home for children whose healthcare needs have increased following an acute hospital admission or Emergency Department presentation. The program goal of the service is to assist tranisioning children and their families from the hospital bact to a home environment. It aims to enhance and maintain the independence of both the patient and their caregiver, ensuring that the child can continue theri recovery and care in the comfort of their home. The program provides vital support to ease the challenges of adjusting to post-hospital care, promoting a seamless and sato improve and maintain a patient/carer's independence within the community, supporting a smooth transition from inpatient setting to home environment.
