An exciting opportunity exists for a Nurse Manager to provide a referral based, person centred assessment and discharge planning service within an interprofessional framework with the aim to provide optimal patient outcomes and optimised patient flow.
Community Connect provides high standards of patient focused care through risk screening, assessment, discharge planning, referral to appropriate support services and facilitating patient transition from hospital to the community. The service supports the broader hospital staff as subject matter experts in respect of available community support services which aim to safe support timely discharge, reduce the likelihood of hospital re-presentation and/or premature admission to residential aged care facilities.

Key Responsibilities include:
1. Implement a model of care that has been designed to manage, prioritise and allocate incoming referrals to ensure timely responsive action, facilitate safe discharges and improve patient flow.
2. Cohesively lead a team of nurses who work in both the acute and community workplaces to ensure optimised care is delivered by enabling access to required community based services
3. Work collaboratively with nursing and interprofessional groups to maximise safe patient discharge opportunities.
4. Provide education and tailored support to support ongoing improvements in patient flow.
Remuneration value up to $164 212 p.a., comprising salary between $5081.10 - $5516.60 p.f., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (Nurse Grade 7) (Fixed-Term Temporary Full-Time Position, 76hrs p.f. up to 12 months).
