About the Role This is a leadership role and supervises one or more Our Way Home Houses for children and young people in Out of Home Care.
This role is responsible for the oversight of the homes as well as the facilitating and coordination of the children's care teams and My Plans.
This role is responsible for managing the Care staff in the houses including Carers and Support Workers.
This role is offered on Permanent, Full-Time basis.
This role is located at our location in Midland and/or Belmont.
If you would be keen to join us, we are now seeking a passionate and committed professional to join our team who can: Provide direct coaching and debriefing to care staff to support their development in the skills required to deliver quality care.Participate in the intake, assessment, and discharge process for assigned children and young people.Develop, implement, review and update comprehensive, holistic and best practice individualised My Plans for assigned children and young people.Coordinate and facilitate a collaborative Care Teams for each child, ensuring that all care team members, inclusive of the child and their family, are either in attendance or having their views represented; and are facilitated in alignment with the Our Way Home 'Care Team' Practice Guide.Maintain case notes, reports, statistical reports and other required documents in accordance with organisational obligations, policies and processes.Consult with internal and external service providers to the child in a timely and professional manner.
Liaise and collaborate with key external agencies and other stakeholders involved in out of home care.
Comply with internal and external agency requirements for incident and other reports.In order to be successful for this role, the applicant will: Formal qualifications in Social Work, or relevant tertiary field.Demonstrated experience working with children and young people who have experienced trauma.Evidenced knowledge and experience in the delivery of Trauma informed care.Knowledge and experience in case management and advocacy for children, youth, and their families.Skills and Experience in staff supervision including performance review and management processes.Strong interpersonal skills, including an ability to communicate with, and respond to, children, young people, and families.Knowledge or Skills in residential care or care planning.Understanding of ecological systems theory.Demonstrated experience working with children and young people from a range of cultures including Aboriginal and CALD children and young people.Parkerville Children and Youth Care are uniquely positioned to provide the successful applicant with the following benefits.
