About the RoleWe're seeking an enthusiastic and experienced hospital pharmacist to lead the Hospital in the Home Pharmacy service and support a team of pharmacists across three sites. Working across Dandenong Hospital, Moorabbin Hospital and Monash Medical Centre (Clayton), the Hospital in the Home Pharmacy team provide innovative and patient-centred care. The team work closely with the multi-disciplinary team to provide clinical pharmacy services in three streams: Anticancer Therapies, Antimicrobials and Palliative and Complex Care. The Hospital in the Home program continues to expand in support of the organisational priorities to support early discharge to appropriate settings and to provide care closer to home. Day in the LifeWorking closely with the multi-disciplinary team, Team Leaders and the Pharmacy Leadership Team, you will lead and develop the clinical pharmacy services in Hospital in the Home and provide expertise in medication management. In this important role you will:Support, guide and work alongside your team members to provide a consistently safe, high quality, efficient and effective serviceCoordinate daily workflow, staffing and prioritiesSupport the training of pharmacy staff and students, with assistance from our team of Clinical EducatorsEnsure relevant key performance targets are met or exceededParticipate in research and quality improvement activitiesEach week you will have a day so that you can focus on team management tasks, developing your team members and honing your own leadership and management skills. A relevant postgraduate qualification, such as Master of Clinical Pharmacy will be highly regarded.
