The Lymphoma Care Coordinator will work with members of the Haematology Unit and Lymphoma Multidisciplinary Team and will be responsible for the coordination of care for our patients in the outpatient setting, requiring care and treatment for Lymphoma across the Hunter New England Local Health District (HNELDH).DUTIES:Provide an expert client-centered consultancy practice participating in direct patient care provision.Provide education on complex clinical issues to patients and carers.Identify and adopt innovative clinical practice models, e.g., implementation and evaluation of new treatments, technologies, and therapeutic techniques relating to CNC specialty.Review, implementation, monitoring, and evaluation of guidelines related to lymphoma treatment regimes.Provide education on complex issues to clients, carers, and families on treatment regimens, complex regimes, relapses, autologous stem cell harvest, and transplant referral pathway.Provision of an expert clinical consultancy resource for the MDT and LMDT team and HNELHD and the ability to work with other care coordinators to provide comprehensive care.Development, review, and evaluation of education resources for patients, carers, and families undergoing treatment.ABOUT YOU:Registered Nurse with current Authority to Practice with APRHA, with demonstrated extensive experience and knowledge of Haematology treatment regimens related to Lymphoma.At least five years post-graduate nursing with 3 years specialty experience in Haematology/Oncology and post-graduate qualification (preferably Masters Level) related to Haematology/Oncology or evidence of working towards the same.Demonstrated ability to communicate clinical concepts and information to non-clinical audiences and provide authoritative advice on clinical issues within the field of expertise.Demonstrated advanced analytical and problem-solving ability to break down complex problems and develop a range of solutions based on specialist expert knowledge of Haematology.Demonstrated capacity to evaluate current clinical practice within Haematology and recommend improvements to avoid potential problems or minimize impacts on the organization.Demonstrated highly developed verbal and written communication skills necessary for documenting research, promoting awareness of current research, and providing clinical services directly to patients/clients.For further information please contact Marissa Ledlin, Operations Manager – Cancer Services on email: Marissa.ledlin@calvarymater.org.auWHY JOIN US?CMN is an Affiliated Health Organisation providing services to both public and private patients primarily from the Hunter New England area.
