Southern Adelaide Local Health Network – Flinders Medical Centre – Rehabilitation, Aged and Palliative Care Salary $88,255.00 - $102,240.00 (Pro Rata) AHP2 | Part Time 31.5 hours per week – Ongoing Join our Southern Adelaide Palliative Services team as a Social Worker today About the role: Reporting to the Manager of Community Palliative Care and the Director of Social Work, this role will provide a comprehensive social work service embracing psychosocial assessment, casework, counselling, advocacy, networking, referral and education to patients, their carers and families, under the care of the Southern Adelaide Palliative Care Service (SAPS), including those admitted to Laurel Hospice. Duties for the role will include: Assessing patients’ abilities, problems and goals using a variety of formal and informal assessments Planning and implementing intervention programs Collaborating with other team members and community agencies Linking patients and carers with relevant community resources and services Documenting assessment findings and interventions About you: High levels of responsibility and organisational skills including the ability to prioritise work, meet deadlines, delegate appropriately Clinical knowledge and experience working with loss, grief and bereavement Experience in the supervision of social work staff and students Appropriate Degree of equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers Proven experience in delivering high quality and safe care consistent with the National Safety and Quality Health Care Service Standards Please see the attached Role Description for additional information.
