About the role
We are seeking a values-driven Case Manager - Homelessness Support Services. The primary purpose of the Case Manager position is to provide case management support outreach to individuals and families with multiple and complex needs who are homeless or at risk of becoming homeless.
This role reports to the Team Leader - Homelessness Support, a permanent part-time position of 60.8 hours per fortnight, located in Seymour, VIC.
You will successfully:
* Review referrals to ensure eligibility, participate in allocation, and perform outreach to meet local targets.
* Conduct initial and ongoing assessments to understand the client's issues, needs, and circumstances.
* Implement tailored case management processes and support clients in setting and achieving personalized goals, including monitoring and reviewing progress.
* Help clients access appropriate services, establish community supports, and advocate for them when needed.
* Identify service gaps and provide feedback to the team leader for potential improvements in service delivery.
You will have:
* A Tertiary qualification in Social Work, Welfare, Community Development, or a related field.
* Demonstrated knowledge and experience in Case Management.
* A current Driver's Licence.
* A current and valid Working with Children's Check.
* A National Police Check, which will be undertaken by The Salvation Army if your application proceeds.
* Experience in Client assessment, including appropriate information, tools, and techniques to gather and analyze information.
* Relevant experience in using appropriate risk models to assess levels of risk, harm, and well-being, including immediate risk.
* Empathy: Strong interpersonal skills with the ability to connect with youth from diverse backgrounds.
* Interpersonal Skills: Strong interpersonal and communication skills, with the ability to build trusting relationships.

Conditions
Compensation is in accordance with SCHADS Award Level 5.
Circa $49.28 hourly + superannuation based on experience level and qualifications, exclusive of penalties, allowances, NFP salary packaging.
