This role requires a social worker who is committed to providing comprehensive psycho-social assessments and complex case management and care planning for patients experiencing acute health challenges. Demonstrated clinical skills and knowledge in psychosocial assessment, counselling, therapy interventions, case management, advocacy and discharge planning. 4. Demonstrated knowledge of crisis intervention, short-term solution-focused intervention and grief and loss. 5. Demonstrated experience and ability to work as a member of multidisciplinary teams in a high-pressure environment with competing demands with sound negotiation and liaison skills. 6. Well-developed time management, prioritisation, computer, written and verbal communication skills. 7. Ability to contribute to research projects, quality improvement and education activities within the designated specialties and social work services. 8.
