About the role
The Clinical Nurse at Activ reports to Clinical Operations Manager and will deliver a high-level of nursing care to our customers. The successful candidate will have advanced clinical assessment skills and contemporary practice including catheter care, diabetes management, complex wound care, PEG management and psycho-social care.
This is a unique position working in our From Hospital to Home base where you will provide support to the service delivery team and to customers transitioning from a hospital environment, as well as supporting our customers in their homes in the Southwest Region.
This is a community-based position required for Monday – Friday day shifts, up to fulltime hours considered.
A typical day in the role may look like:
9. Provide effective communication and guidance regarding clinical practices across the organisation.
10. Provide customer assessment, planning, implementation and evaluation of nursing care in collaboration with individuals to improve health outcomes for customers.
11. Participate in quality and continuous improvement activities.
12. In collaboration with the Clinical Operations Manager, identify and manage the follow up of clinical corrective actions.
13. Support and liaise with customers, carers, colleagues, quality and service delivery teams, external agencies and the public and private sectors to provide coordinated multidisciplinary care.
14. Practices within relevant legislative and regulatory requirements in accordance with organisational policy and guidelines.
15. Participates in case conferences as appropriate.
The ideal candidate will have:
16. Demonstrated advanced clinical skills and knowledge in the delivery of evidence-based nursing care.
17. Highly developed interpersonal and verbal and written communication skills reflected by problem solving, negotiation, conflict resolution and report writing.
18. Demonstrated advocacy for customers, families and carers within a multi-disciplinary team framework to deliver person-centred care.
19. Knowledge and understanding of continuous quality improvement, clinical audits and accreditation processes
20. Knowledge of NDIS requirements and relevant legislation
21. Experience and knowledge in using health management IT systems
22. Current AHPRA registration in the category of Registered Nurse by the Nursing and Midwifery Board of Australia
