We are here for everyone and believe that all people have the right to be respected and their differences celebrated.About the roleThe Care Manager works within the In Home Support programs providing care and services to elders living in the community. The programs under In Home Support include Commonwealth Home Support Program (CHSP), Home Care Package program (HCP), Veterans Home Care (VHC) and private services.The Care Manager’s responsibility is to ensure that the elder’s quality of life is enhanced through their relationship with MiCare. The position is pivotal in facilitating this through their commitment to person-centred case management, client support and astuteness in assessment, care planning and review.Undertake initial assessment, care planning, ongoing review and monitoring of elders as outlined in MiCare Home Care Policies and in accordance with the Commonwealth Government Aged Care guidelines and Charter of Aged Care Rights.Liase closely with internal and external stakeholders to ensure the elder receives services appropriate to their needs and in accordance with program guidelines.Initiate case and planning meetings as required, ensure solid documentation and follow through on elder health, care and service delivery.Develop an individualised budget for the elder with a Home Care Package, to ensure spending compliance within the Home Care Package Guidelines.Responsible for organising referrals and permanent placement where required.Provide support and advocacy for elders and carers as required.Be actively involved with quality review for the programs ensuring your documentation and processes meet compliance.Develop and maintain working relationships with relevant agencies and networks to promote MiCare.Respond to phone and online enquiries, public education to potential clients.About youDemonstrated knowledge of sound case management within the aged care sector, preferably working with complex family and social issues.Clear understanding of the Aged Care Quality Standards.Solid understanding of Commonwealth Government community aged care programs (Commonwealth Home Support Program, Home Care Package program and Veterans Home Care).Proactive, reliable, with excellent communication skills and attention to detail.Ability to work with others and be a participative and supportive team member.Ability to meet compliance requirements of the role such as Current First Aid and CPR certificate, valid QLD drivers licence with an insured and registered vehicle, ability to provide evidence of current flu and COVID-19 vaccination.QualificationsTertiary qualification in Allied Health, Nursing or Social Work.Why MiCare?Family friendly, work/life balance workplace.Hybrid client facing role.Generous salary packaging benefits for the non-profit sector.Rewarding and meaningful work.For enquiries please contact Glenda Kerr on glenda.kerr@micare.com.au or 0434 681 160.MiCare is a Child Safe organisation and is committed in everyday practice to ensure the safety and wellbeing of children.MiCare says “NO” to Family Violence.
