Your role within our Veterans Connect service will allow you to:
Assist Veterans and their families to connect to services and supports Build relationships with Ex-Service Organisations and community service providers to ensure strong referral pathways for Veterans and their families Joining us, you'll be part of a team of dedicated individuals focused on providing evidence-based best practice in Veteran-focused care coordination.
Navigating access to the right services and overcoming barriers in accessing care and support, your work will be diverse and impactful.
Job Description While you will be primarily based in our Gosford office, our Maitland office will serve as a secondary base location.
Veterans Connect covers both the Central Coast and Hunter regions, so some travel is to be expected with the use of our pool cars.

The successful candidate will have:
Demonstrated experience in care coordination or case management across assessments and planning activities in health, community, or Veteran care settings Understanding of enabling factors and barriers to accessing services for Veterans Cert IV / Diploma or other tertiary qualifications in Community Services sector OR relevant lived experience Working with Children and National Police Check are mandatory requirements for all staff.
