Hospital Pharmacist Grade 2 - Rotational

About this role
This role involves the provision of high quality clinical pharmacy services on a rotational basis, working in conjunction with the team of pharmacist and technicians at GH Horsham. Specific duties include:
* Provision of ward/unit based clinical pharmacy services to inpatients in areas such as acute medical and surgical, subacute, ED and ICU
* Dispensing services, including credentialing for chemotherapy dispensing
* Provision of medication-related advice and education to clinical staff and patients
* Commitment to the provision of a high quality, safe, efficient and effective pharmacy service ensuring continuum of patient care
Professional development is supported through regular in-service education including journal club meetings and support for seminar/conference attendance. In addition, professional support is provided through team-based structures as well as formal education programs including AdPha residency programs.
Key responsibilities include active contribution to:
* Provision of comprehensive clinical pharmacy services (assessment, intervention, advocacy and consultation)
* Mentoring and clinical supervision of learners at all levels including pharmacy students, interns, residents and other Grampians Health staff
* Multidisciplinary team meetings and quality improvement initiatives
Skills and experience
* Currently hold or be eligible for general registration as a pharmacist with AHPRA
* Prior clinical pharmacy experience in a hospital is preferred
