About the Role
This is a part-time or full-time contract position based in Gladstone, providing a supportive and flexible working environment with no shift work or after-hours duties.
As a Clinical Care Coordinator, you will collaborate with the Clinical Lead and Operations Manager to provide essential services throughout the Country to Coast Queensland (CCQ) region.
* Support clients with severe mental health conditions by delivering direct treatment and coordinating their care effectively.
Responsibilities
* Conduct thorough clinical assessments and develop tailored treatment plans
* Deliver evidence-based psychosocial interventions
* Manage a clinical caseload and conduct risk assessment with management plans
* Engage in data collection and contribute to monitoring and evaluation
* Collaborate with multidisciplinary teams and external stakeholders
* Facilitate care planning and review processes
* Monitor and escalate mental health needs as necessary

Qualifications, Experience and Skills
* Current annual practicing certificate from AHPRA
* Tertiary qualification in psychology, social work, mental health nursing, or a relevant discipline
* Experience with complex mental health
* Experience working in cross-cultural settings, particularly with First Nations people
* Experience working in trauma-informed and recovery-oriented service
* Ability to work in a team within a diverse environment
* Strong problem solving and analytical skills
* Excellent organisational and time management skills
* Excellent verbal and written communication skills
* Proficiency with Microsoft Office and software
Requirements
* The successful applicant will have or be willing to obtain Driver's Licence
* Police check
* Working with Children Check (blue card)
