You will play a vital role in identifying the needs of older adults and connecting them with services that support their independence and quality of life.
Key Responsibilities:

Requirements:
* At least 1 year of experience in aged care, including client assessments and/or service delivery.
* Tertiary education in a health-related discipline, such as nursing, occupational therapy, physiotherapy, or social work.
* Current unrestricted General Registration with AHPRA or a relevant professional association.
* Proficiency in Microsoft Office applications, including Word, Excel, Outlook, and client management systems.
* A current Australian driver's licence and a comprehensively insured vehicle for travel within your service area.
