The organisation provides a broad range of primary health care and aged care services, and we are now the largest Aboriginal Community Controlled Health Service provider by land area in Queensland. Their vast service footprint spans approximately 640,000 km2, including 11,000 Indigenous people. Their service includes two Hospital and Health Service regions (Central West and Northwest) and shares its boundary with the Western Queensland Primary Health Network (WQPHN).
Position Summary:
The position is responsible for providing pharmacy-related support to clients of the Service in the quality use of medicines and pharmacy-related services in order to promote, maintain, and improve their health with a focus on the organisation’s model of care to improve the health and wellbeing of Aboriginal and Torres Strait Islander Patients in a culturally sensitive manner; demonstrating high levels of professionalism and integrity and applying high moral and ethical standards to ensure confidentiality is maintained at all times.
Key Performance Indicators:
o Accountable for or assisting in developing, implementing, monitoring, reviewing, and evaluating the Organisation’s Clinical Service Plan;
o Ensure KPIs relating to your area of responsibility are being met;
o Provide medication advice and education services to the clients of the health service according to the policies and cultural practices of the health service;
o Provide Home Medication Reviews for clients;
o Contribute to existing programs of chronic disease management in the health service to expand the capacity of patients to manage their own conditions through quality use of medicines;
o Provide direct assistance to ensure safe and high-quality systems are in place for the transport, stock management, storage, dispensing, and resupply of pharmacy items in all clinics;
o Provide and/or coordinate support services, programs, and pharmacy related professional support services in all clinics;
o Assist all clinics to create strong partnerships with local quality retail pharmacy providers;
o Work directly with local retail pharmacies and Pharmacists where required to broker access and support for both clients and services, including assisting mainstream pharmacies to access cultural training and enhancing knowledge and implementation of CTG measures for clients;
o Contribute to the development and implementation of clinical policies, procedures and protocols that enhance clinical quality and safety;
o Work alongside AHWs/AHPs, RNs, and GPs, delivering one-to-one medication education and support for complex clients where required, and utilizing the opportunity to transfer skills and build workforce capacity at the same time;
o Provide timely advice to managers where medication issues potentially impacting clinical safety and quality are identified
o Undertake other duties within your scope of practice and must make all attempts and endeavours to improve the service(s) that you deliver or help deliver as directed by your Supervisor or allows you to perform your positional roles and responsibilities efficiently and effectively.

o If nominated in the Delegations Register, Succession Plan or as an Identified Leader, accountability for tasks and responsibilities
o Use and contribute to Quality Management System (QMS) (report of closed, outstanding tasks at Appraisal)
o High standard of records management (data entry, data cleansing, archiving, reporting) to meet regulatory, standards and/ or reporting requirements and timeframes.
o Attend internal and external meetings as nominated/required and sharing of key information
o Communication on key matters to your area(s) of responsibility that may have an impact on the organisation
o Contribution to local and whole team including undertaking additional duties as directed by Supervisor.
Qualifications and Experience:
Position Specific:
o Tertiary Education i.e. Bachelor qualification in Pharmacy or equivalent;
o AHPRA Registration as a Pharmacist in Australia;
o Accreditation to perform Home Medicine Reviews;
o Registration with External Certifying Body: Registration with Australian Health Practitioners Regulation Agency (AHPRA); registration with Medicare where eligible for a provider number.
Other Requirements:
o First Aid and/or CPR Certification.
o A current Queensland C class open driver's licence; and
o Working with Children Suitability Notice (Blue Card)
o A current Police Check.
o Up to date COVID-19 Vaccination status (min requirement of 3)
