Located just minutes from the Albany town centre, our facility is set amongst peaceful and natural surroundings.Whilst working in this rewarding role you will be a valued member of our multidisciplinary team and provide quality and resident-focused Occupational Therapy services. You will work in partnership with our residents and their significant others to conduct relevant assessments, determine individual capabilities/preferences, and set realistic goals that will support the maintenance or restoration of independence.This will be a perfect opportunity for you if you have a passion for your work and enjoy making a positive difference to the lives of others. You will also play a valuable role in our Transition Care Program which may involve some home visits in addition to residential work.The Juniper Transition Care Program is a goal-oriented, strength-based, reablement program. The program goals are created in partnership with the customer and family, and outcomes are measured against the progress achieved towards goals. You will play an important role in the customer journey by designing and implementing interventions tailored for the customer. Care plans are revised frequently and collaboratively with the multidisciplinary team to support progress towards independence.This position is being offered on a permanent part-time or full-time basis, with up to 75 hours per fortnight available.
