The Preventative Integrated Care Service (PICS) will provide a rapid response (review within 24-48 hours) for intensive sub-acute medical management, delivered by a specialised medical, nurse practitioner and allied health team with a disease specific focus through flexible and varied modalities.

The Clinical Nurse is a registered nurse who is accountable for practice at an advanced level, working collaboratively within the unit and with other members of the health care team including consumers/carers, other health professionals, general practitioners and other agencies (including non-Government agencies). This role provides the delivery of clinical care and support to the Preventative Integrated Care Service within the Preventative Health Division.
The Clinical Nurse provides contemporary clinical leadership and care within a specific area supporting the general function of the unit through the application of best practice principles, education, leadership and portfolio management in the application and role modelling of professional nursing practice standards in accordance with the Nursing and Midwifery Board Australia (NMBA).
About you
Our ideal candidate will be someone who can demonstrate the following:
* Demonstrated advanced clinical knowledge in the area of chronic condition management delivering expert nursing care in complex situations while providing support and direction to other team members.
* Demonstrated ability to role model excellent standards of comprehensive, evidence-based nursing care and documentation to facilitate quality patient outcomes, in accordance with National Safety and Quality Health Service and professional standards.
* Demonstrated ability to communicate effectively and maintain a positive workplace culture through role-modelling well developed consultation, liaison, negotiation and conflict resolution skills, engaging professionally with members of the healthcare team, patients, relatives and visitors.
* Ability to prioritise clinical and related administrative duties, including effective and efficient planning and organisation of complex caseloads with proven ability to use Virtual Care, digital health/information and reporting systems appropriate to the position.
Mandatory Requirements:
* Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16.
