The Hobart Continuing Care Team is a dynamic and supportive multidisciplinary team in the Hobart and southern Tasmania area. The team follows contemporary practices in providing longer-term treatment, care coordination, community, and extended care rehabilitation for individuals requiring assistance in developing functioning skills. The team works in collaboration with all areas of the mental health sector and Primary Health. The multidisciplinary team provides care coordination, psychological interventions, crisis management, and service coordination.
The Role:
As part of the Mental Health Services Team, facilitate, co-ordinate and deliver high-quality consumer-focused mental health services in accordance with the National Standards for Mental Health Services, Agency policy and direction, legal requirements, and relevant professional competencies of the Peer Worker Carer will:
* Support the key tenets of the Operational Service Model (OSM) for the Hobart Continuing Care Team, which are to provide a supportive and therapeutic environment that promotes independence, opportunity, and recovery.
* Act as a positive role model to support carers and families to determine and access the range of services they require.
* Provide outcome-focused, recovery-oriented services to carers and families of consumers of the mental health service.
* Use their lived experience to share experiential knowledge, skills, and strategies to support carers to navigate the mental health system and social services system and optimise outcomes for friends, families, and individuals.
You will need to:
Provide support to families and carers of people receiving support from public mental health services, in identifying appropriate supports (community resources and services) and ensuring families and carers are aware of their rights and responsibilities.
Attend multidisciplinary team meetings and participate in discussions with a focus on family/carer rights and responsibilities, quality improvement initiatives, planning, and problem-solving in line with mental health and recovery frameworks.
Develop and maintain positive relationships with internal and external stakeholders, and relevant Lived Experience networks.

Permanent full-time, day worker position, working up to 76 hours per fortnight, commencing as soon as possible.
* Notwithstanding hours to be negotiated with the successful applicant.

Salary:
$67,193 to $70,993 per annum. Our Employer 11.5% superannuation contribution is on top of this amount.
* Salary range is in accordance with Public Sector Unions Wages Agreement 2022.
