Care Manager (Home Care Packages) Part Time / 25 hrs per week We are BCR Communities – a not-for-profit award winning community organisation serving communities within the Illawarra and Shoalhaven for over 30 years (formerly Bay & Basin Community Resources). We are seeking experienced Care Managers to join our team to provide person centred supports for clients receiving Home Care Packages (HCP). This role is part time and based predominantly in our South Nowra and Albion Park offices with the option of a hybrid work from home arrangement. The role also involves visits to clients in their homes and in community, and the initial orientation and training will take place in South Nowra. The role requires availability Monday - Friday, 5 hours per day. You will be supporting clients to develop healthy ageing and wellness goals in their care plans, in accordance with the Aged Care Quality Standards. You will achieve this through working in a multidisciplinary team, forming strong professional relationships with clients and their support network, whilst working closely with all suppliers and health professionals involved in their care. We are specifically seeking someone who will bring a track record of supporting ageing clients with diverse and complex care needs, including mental health and dementia-related behavioural aspects.
