Grampians Health Horsham is looking for a passionate Social Worker to join our forward-thinking team.
This full-time, permanent role with a monthly ADO, offers the opportunity to work in our Emergency Department and acute wards whilst also providing education and skills development to our Urgent Care Centres in Stawell and Edenhope.
As part of our Social Work team, you will play a vital role in enhancing the psychological and social well-being of our patients.
You will ensure that each patient's social, emotional, and cultural needs are met, while working to minimize the impact of illness and hospitalization.
Key responsibilities include conducting psychosocial and risk assessments, facilitating complex discharge planning, and providing support to both patients and their families.

Key Selection Criteria Essential Degree in Social work.
Eligible for full membership in the Australian Association of Social Workers (AASW).
Current Driver's Licence.
Experience in an acute health setting and/or in the areas of Alcohol and other Drugs, Family Violence or Child Protection.
Proficient use of information technology.
Experience in dealing with the following systems: Child Protection, The Orange Door, ACSO.
Experience in the supervision of students and/or Social Workers.
Minimum 2 years' experience in an acute health setting and/or in the areas of Alcohol and other Drugs, Family Violence or Child Protection.
