The Care Management Team Leader is a senior member of the organisation.
You would be responsible for successfully directing and coordinating all activities associated with care management including the care of potential and existing clients.
This role provides leadership and effective management of the care managers and overall client experience including the delivery of quality client care services and business development in line with the strategic plan and annual goals.

Our client is seeking a dedicated Care Manager Team Leader to join our Carnegie office and oversee a team of 5 care managers.
Key Responsibilities: Lead and mentor a team of care managers, providing guidance on clinical processes and care plan reviews.
Ensure compliance and conduct clinical assessments to maintain the highest standards of care.
Work closely with the Business Operations Manager and adapt to changes within the organization.
Engage with clients and their families, ensuring exceptional service delivery across the Bayside area.
Ideal Candidate: Bachelor's degree in a related field and or equivalent combination of education, equivalent to or greater than the Care Management Skillset.
Experience in managing and delivering community services (preferred).
Experience in Quality Management and Compliance including a strong understanding of the Aged Care Quality Standards Highly efficient and organised with proven time management skills.
Strong engagement, influencing and negotiating skills which can be applied in a relationship management and sales arena.
An effective communicator with excellent customer service skills and demonstrated problem solving abilities.
Able to demonstrate your ability to work both autonomously and as part of a team.
Self-motivated and outcome orientated, with the capacity to deal with multiple and at times conflicting priorities.
Must possess a valid driver's license.
