Job tasks and responsibilities
Using a team approach you will assist the Home Care Package Nurse or Health Professional Coordinator with the provision of support and assistance to clients in the coordination and management of their individualised Home Care Packages.
In collaboration with the HCP Nurse/Health Professional Coordinator:
* Assist with the Coordination of Home Care Packages
* Assist with service intake, coordination of home care and support, client agreements, support planning, referral and care-management.
* Identification of consumer's needs and goals.
* Development of client directed goal-based support plans incorporatinga wellness approach.
* Facilitating the introduction of support and/or care workers to the consumers
* Monitoring of services that are provided to ensure consumers' needs and goals are being met.
* Contributing to a multidisciplinary team approach.
* Liaising with referral agencies, service providers, consumers, family/advocates to ensure best possible outcomes for consumers.
* In partnership with the HCP Nurse or Health Professional Coordinator and consumer, monitor and review consumer progress and changes in health & wellbeing status.
* Accurate and timely documentation and maintenance of all Consumerrecords.

About you
You are an enthusiastic and dedicated Professional with a real passion for improving the lives of our consumers and supporting a team of health professionals to deliver quality consumer care and support. With the ability to work autonomously, you will thrive in this multidisciplinary environment. You are adaptable, enjoy meeting new people and working in a variety of settings.
* Certificate IV or Diploma in Community Services or relevant discipline.
* Experience working in an aged, health or community setting.
* Current motor vehicle driver's licence.
* Two (2) professional referee checks - worked with candidate recently.
* National Police Certificate obtained within the last three (3) months.
* Working with Vulnerable people card.
Highly Desired Experience
* Five years working in aged, health or community care setting.
* Working with consumers, carers and family to achieve goal oriented, care planning and coordination resulting in appropriate consumer outcomes.
