Funded by the Department of Child Safety, Seniors, and Disability Services (DCSSDS) through the Extended Post Care Support and Next Step Plus initiatives, we focus on ensuring young people are prepared to achieve positive, long-term outcomes as they move into adulthood. Our team collaborates with a wide range of stakeholders to help young people develop skills, access resources, and plan for their future. We are looking for a driven, initiative-taking individual who can work both as part of a team and independently. Our Transition to Adulthood programs are evidence-based, structured, and responsive to the unique needs of young people leaving care. Your Role In this role, you will be a key member of the Transition to Adulthood team, reporting directly to the Team Leader. You will bring your expertise in assessment, crisis intervention, and therapeutic approaches to support young people as they navigate the transition from out-of-home care to adulthood. About you Are you passionate about helping young people reach their potential and build strong community networks? We are seeking a candidate with experience in case management who is dedicated to supporting young people during their transition to adulthood. You understand the profound impact mentorship can have on a young person's life. You are values-driven, have a growth mindset, and possess the following skills and qualifications: Delivering effective coaching, care coordination, and advocacy for young people transitioning from out-of-home care Collaborating with key stakeholders, such as natural mentors, Child Safety, health providers, and community support services, to create a positive experience for young people Mentoring young people to help them set and achieve personal goals, finding creative solutions to overcome barriers Providing practical support in housing, employment, health (including mental health), and overall wellbeing to establish a foundation for growth Working sensitively with Aboriginal and Torres Strait Islander people and communities to ensure culturally inclusive services in a safe environment. Qualifications and Experience To be successful in this role, you will need: A degree in Social Work, Psychology, Social Sciences, or a related discipline A minimum of two years' experience in a human services role, ideally within a multidisciplinary team A solid understanding of human services and care coordination Knowledge of the Child Protection Act 1999, or a willingness to learn Experience in trauma-informed care principles, particularly in assessment, planning, and decision-making for young people Pre-Requisites National Police Certificate (or ability to obtain one) Working with Children (Blue Card) (or ability to obtain one) Child Safety and Personal History Screening Check (LCS) for roles providing care in a licensed care service Current driver's licence Importantly, you will be someone who closely aligns with our organisational values of: Unconditional Love, Continual Innovation, Mutual Trust, Wise Stewardship and Safety.
