Be the first internal Occupational Therapist at a small but growing provider of home care and disability services that embraces innovative care strategies.
The organisation has clients throughout Perth's Southern suburbs, striving to keep clients happy, healthy and independent in their own homes while connected to family, friends and the wider community.
You will drive growth by transitioning Occupational Therapy services from a brokered arrangement to an in-house offering. Key responsibilities include:
* Designing and implementing new systems and templates, including assessments
* Visiting clients in their home to apply specialist knowledge and empower them to maximise independence and improve well-being
* Developing, implementing, evaluating and adjusting comprehensive, holistic and progressive care plans
* Building productive relationships with clients and involving them in decision making processes
* Prescribing equipment and recommending home modifications
* Delivering advice, education and training to clients and their families

To be successful, you will have:
* Full registration as an Occupational Therapist with AHPRA
* Experience as an Occupational Therapist in home care or disability with an understanding of funding models
* Demonstrated ability conducting thorough assessments and creating holistic care plans
* Confidence working autonomously and using initiative
* Excellent interpersonal and customer service skills to build rapport with clients and families
Salary range is $110k - $120k + super.
