This full-time, permanent role based in Launceston offers the opportunity to work intensively one-on-one with young people and their families, supporting them in improving their mental health, wellbeing, and functioning. The position commences in late May and involves close collaboration with the Child and Youth Mental Health Service (CYMHS) in Northern Tasmania, providing intensive support packages of care to young people with complex mental health issues. The Position As part of a highly skilled and supportive outreach mental health team, you will provide comprehensive care planning, risk assessment, and therapeutic one-on-one support. Working alongside the CYMHS clinical team, you will use evidence-based therapeutic modalities to deliver clinical case management and intervention. Your role will involve community-based and outreach work to meet the needs of young people across Northern Tasmania. Key Responsibilities: Deliver evidence-based psychosocial and psychological interventions to young people and families with multiple needs. Monitor and review client progress against care plans and outcome measures. Engage in case discussions and reviews with partner organisations and care teams. Support clients in maintaining or re-engaging in education, employment, social and recreational networks, self-management, and independent living. Collaborate closely with CYMHS and other services to provide wrap-around support. Develop exit or transition plans for clients, ensuring ongoing support through referrals where appropriate. Key Experience: Bachelor’s Degree in a relevant Allied Health profession (Psychology, Social Work, Occupational Therapy, or equivalent) with 2-5 years of post-graduate mental health experience. Experience delivering therapeutic support to young people aged 12-18 years and their families. Strong capacity to work autonomously in the community and as part of a multi-disciplinary team. Highly developed analytical skills with experience in developing care plans and psychological interventions. Ability to collaborate with mental health services, GPs, Psychiatrists, and other statutory or community-based services. Full registration or eligibility for registration/membership with the relevant professional body. Enthusiasm for working in trauma-informed, youth-friendly ways. Willingness to provide support through outreach in rural areas and via e-health platforms.
