About the role
An exciting opportunity for a skilled and experienced Registered Nurse exists to share the position of Clinical Care Coordinator - Cardiac unit at Calvary Adelaide Hospital. The applicant/s could work from 0.3-0.4 in a job share capacity with work days to be considered, and to cover annual leave as required. This position requires dual reporting to the Cardiac Clinical Manager and the Clinical Care Coordinator Manager. This key role is responsible for coordinating complex discharge planning in accordance with health fund DRG’s and desired length of stay, as well as undertaking cardiac discharge education and making appropriate referrals to outpatient rehab programmes.
The successful Registered Nurse will have a comprehensive knowledge and understanding of cardiac surgical and interventional procedures, as well as expected length of stay. As well as demonstrating effective working relationships with Visiting Medical Officers, Managers and Nursing and Allied Health teams.
Applications are invited from Registered Nurses who have a minimum of 2 years post qualification acute experience who wish to develop skills in case management and discharge planning.
About you

o Registered with APHRA, and with a minimum of 2 years post-graduate experience in acute care hospital nursing
o Cardiac acute nursing experience is required
o An understanding of health funds is highly desirable
o Demonstrated advanced interpersonal and organisational skills and a high level of analytical, communication and reporting skills
o Demonstrated effective working relationships with Medical Practitioners, Managers, and all staff
o Strong communication, interpersonal, organisation and decision making skills
o Ability to build positive relationships with patients and their families
o The ability to work autonomously
o Demonstrated commitment to excellence in customer service and continuous quality improvement
o Commitment to the Mission and Values of Calvary with patient-centred quality care as the primary focus of service
