As a Registered Nurse at Tamworth General Practice, you will play a pivotal role in delivering high-quality care and support to our diverse patient population.Your responsibilities will encompass but not limited to:• Conducting patient assessments, creating care plans, and health screenings with focus on GPMP, TCA, 715, and 75+ health assessments.• Administering medications, vaccinations.• Perform chronic disease management, including monitoring patients with diabetes, hypertension, and other chronic conditions.• Maintain accurate patient records and assist with data collection for quality improvement initiatives.• Supporting GPs.
