About The Role
In this role, you will work closely with patients to offer emotional and social support as they manage chronic cardiac and pulmonary diseases. You’ll engage with individuals from diverse socio-economic backgrounds, ensuring they feel supported throughout their rehabilitation journey. Patient referrals may come from a wide area, spanning from Scone to Kurri.

About You
* Experience in chronic disease management is ideal.
* Strong communication skills – both verbal and computer-based.
* An ability to connect with a diverse patient population.
Requirements
* Bachelor in Social Work or equivalent with eligibility for membership with Australian Association of Social Work (AASW)

* To be eligible for permanent employment in this position you must be an Australian Citizen, or a permanent resident of Australia, or a New Zealand citizen with a current New Zealand Passport.
