Position Overview: As a Social Worker, you will have the opportunity to operate under limited supervision within a multidisciplinary team. Your primary responsibility will be to deliver a high-quality service to patients in the Emergency Department. You will be accountable to the Team Coordinator of the ED Social Work Service. Utilise a trauma-informed care model to provide assessment, consultation, and intervention. Collaborate with community agencies to enhance effective discharge planning. Prioritise consumer participation by focusing on patient strengths, empowerment, and well-being.
