About the role
As a Registered Nurse in a busy and dynamic GEM/REHAB@HOME service, you will be expected to provide nursing care to patients in their own homes.
The GEM/REHAB@Home Registered Nurse maintains the continuum of care delivered within the community setting. A Nurse in this service functions as a sole practitioner and is required to provide both safe and competent care as part of a multidisciplinary team, to coordinate and provide an expert clinical service at an advanced level, in line with relevant standards, guidelines and policies of Eastern Health. This care may take the form of medication management, wound care, assessment, observations including bladder scans/ECG/pathology etc.
You will be part of a large multidisciplinary team including Geriatricians, Specialist Rehabilitation Physicians, OT, Physiotherapists, Allied Health Assistants, Speech Pathology, Dieticians.
About you
To be successful in this role you will possess:
* Minimum of 5 years acute medical/surgical post registration nursing experience or nursing specialty equivalent e.g. ICU/ED/CCU/Sub Acute/medical/Community
* Current Division 1 Nursing registration with AHPRA
* Demonstrated initiative and self-direction
* Willingness to maintain and advance knowledge & skills relevant to this position
* Comprehensive systems-based patient assessment skills and documentation of findings
* Working knowledge of wound management (acute and chronic)
* Urinary catheterisation - male & female
* Pathology result interpretation
* A strong commitment to high quality care
* The ability to work as part of a diverse team
* Excellent communication and organisational skills
* Current Victorian Drivers Licence
Vaccination against infectious disease is a mandatory requirement of this role. An offer of employment is conditional on you providing evidence that you are currently vaccinated against COVID-19 with booster & Influenza, prior to commencing employment.
