The Single Assessment Services is a multidisciplinary team of Clinical and Non-Clinical staff whose main function is to assess frail clients 65 years and over and Indigenous clients 55 years and over for simple Assessment for Commonwealth Home Support Program codes, permanent residential aged care, respite residential aged care, home Care Package Program in the community, transition Care Programme, short Term Restorative Care and once care is approved SAS refer to community service providers to provide this care.
About the role: The Home Support Assessor works within the Single Assessment Services providing simple home-based face to face assessments of clients referred via the My Aged Care Gateway using the Integrated Assessment Tool (IAT) under the single assessment system. Working with the client, the Home Support Assessor establishes goals specific to the client; develops appropriate strategies and actions, including referral to aged care and non-aged care services; and develops a Support Plan. Completing the IAT and matching referring clients to appropriate service(s).
About you: Certificate IV in Health Services Assistance / Allied Health Assistance or Certificate III in Home Care or Aged Care or equivalent work experience is highly desirable. High level interpersonal and communication skills. Great organisational skills with the ability to prioritise. Proven ability to work effectively as a member of a team with an ability to facilitate, promote and adapt to constant change.
